Position Competencies:
● Ability to relate to individuals who have severe impairments in their ability to function in the community.
● Ability to respond quickly, effectively and with equanimity to member crises and life situations.
● Ability to work as a team member and to work with other agencies, community resources and member’s families.
Position Requirements:
Must have MSW or MFT degree; BBS supervised post-master’s hours may be available for this position for unlicensed candidates.
Must have at least one-year experience working with behavioral health populations including homeless persons with co-occurring disorders. Must be experienced working with low income, diverse populations including persons affected by mental illness, substance use and incarceration. Subject to California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.
● Ability to demonstrate adequate literacy skills to perform work duties will be considered.
● Must have a valid California Driver's license and the availability of a car with adequate insurance.

Primary Duties:
● Program Coordination and Oversight: Lead the coordination of all aspects of the ECM, ensuring seamless integration with community support providers.
● Oversee the provision and execution of ECM services, ensuring adherence to the highest standards of care.
● Member Liaison: Serve as the primary contact for members, facilitating a trust-based relationship through open communication and personalized attention.
● Provide face-to-face services and direct care at members' preferred locations, ensuring they feel safe and supported.
● Care Plan Implementation: Oversee the development and implementation of personalized care plans, steering members towards their health and wellbeing goals.
● Connecting Members to Social Services: Facilitate connections to vital social services and supports, including organizing transportation arrangements as necessary.
● Advocacy: Champion the needs and preferences of members and their families, advocating fiercely on their behalf within the healthcare system.
● Specialized Care Approaches: Adopt and promote the use of motivational interviewing, trauma-informed care, and harm reduction approaches in member interactions.
● Hospital Discharge Coordination: Coordinate with hospital teams to facilitate smooth discharge planning, ensuring a seamless transition for members back into the community.
● Office Visit Accompaniment: Accompany members to office visits, providing moral support and assistance to ensure a fruitful visit.
● Monitoring and Training: Diligently monitor treatment adherence, assisting members in navigating any barriers to successful compliance.
● Develop and conduct health promotion and self-management training programs to empower members in their health journeys.
