Manhattan Medical Arts is expanding our outpatient practice in West Village Manhattan and Forest Hill, Queens, NY. We are seeking a Full-Time Family Medicine or Internal Medicine Nurse Practitioner to work in a 100% outpatient care clinic. In this position, the NP is a co-clinical lead of the integrated care team that will plan, coordinate, deliver and manage comprehensive healthcare services for patients. Ensures patient/provider continuity and access to timely and appropriate levels of care.

Duties:

• Provide complete, comprehensive, family-oriented outpatient care to their assigned panel of patients through the life cycle.

• Provide preventive care, manage acute and chronic conditions, and develop and coordinateinated care.

• Make appropriate referrals for additional services.

• Provides consultation and supervision to mid-level providers.

• Communicate with internal and community staff such as care coordinators, care managers, and other agency staff involved in the patient's care plan.

• Performs other duties as assigned.

NP Requirments and Skills:

• Graduate of an accredited School of Nursing.
• Licensed NP (Nurse Practitioner) to practice in New York.
• DEA license
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized
• Able to multitask efficiently and effectively
• Solid assessment, clinical, and documentation skills
• Patient-oriented

The position requires working some morning shifts, some evening shifts, and some weekend shifts.
