St. Luke's Health Partners is playing a large role in our ability to deliver coordinated, affordable, accessible care.Health Partners supports a healthy work-life balance and offers a dynamic environment whereby team members work cross-functionally and collaborate to drive its value-based care strategy. St. Luke’s Health Partners Team engages with the value-based patient population to provide patient centered care management utilizing best practices to achieve better health outcomes, lower total cost of care, and improve patient experience. The team consists of registered nurses, social workers, pharmacist, registered dieticians, certified diabetic educators, respiratory therapist, community health workers, and outreach coordinators working together to support patients.

What you can expect from this role

  • Work Schedule: Monday-Friday, business hours

  • Full Time with the option of work from home, hybrid or on site

  • Medical/Dental/Vision - Start the 1st of the month following your start date

Qualifications

  • Education: Accredited program in alignment with licensure requirements

  • Licenses/Certifications: Current Registered or Licensed Dietitian in Idaho
